When you or your loved one needs primary care services and cannot or choose not to travel to a doctor's office, there is a new way of receiving clinical services in your place of residence. As the population ages, illness, functional limitations and transportation can make it difficult to get to doctors' appointments. This can result in missed appointments, and often lead to emergency room visits or unwanted hospital stays.
Our services include:
- Wellness and preventative care
- Managing chronic illness
- Assessment, evaluation, diagnosis and treatment for illnesses as medically necessary
- Ordering and interpreting diagnostic testing such as lab work, x-rays and EKG’s
- Diagnosing and treating acute and chronic conditions such as diabetes and hypertension, congestive heart failure, pulmonary disease, infections and injuries
- Counseling patients and their families on medical conditions and disease processes
- Educating patients and their families on disease prevention, health and lifestyle choices
Our medical staff
Our medical team is lead by our in-house physician medical director and staffed with certified registered nurse practitioners who collaborate with the medical director on all patient visits.
